ABSTRACT INFORMATION

ATTENTION!

The deadline for submission of abstract has been prolonged up to 5^th March 2004.

• The abstracts submitted must not have been published or presented previously at any Polish Cardiac Society conference. They should be written in Polish or English.

• The deadline for submission of abstracts is February 29^th 2004 (postal date). Abstracts sent after this date will not be accepted.

• Abstracts should be sent by regular mail to the Organizing Committee (address as printed on the original enclosed submission form) or sent by on-line (submission form is to be found on internet site: http://www.abstrakty.ptkardio.pl).

• Abstracts sent in a different manner (fax, not original submission forms etc.) will not be accepted.

• Additional original submission forms can be obtained from the Secretariat of the Organizing Committee of the Congress.

• We kindly request that abstracts should not be sent by regular mail and on-line simultaneously.

• Each submission should contain:
  a. for abstracts sent by regular mail:

  - abstract presented on original submission form with address of corresponding author and indication of subject category included,

  - a floppy disc containing the abstract or 4 photocopies,

  - completed acknowledgment card for Confirmation of receipt of the abstract by Scientific Committee,

  - completed returnable form for Notification of the abstract's eligibility.

  b. for abstracts sent on-line:

   - abstract,

   - e-mail address of corresponding,

   - indication of the subject category.
